Before your first visit, please download the appropriate packet of forms. Print them out and bring the completed forms to your scheduled appointment to help us stay on schedule.

Records Release
If you would like to have your records transferred, either to yourself or another doctor,
we must have a signed release from you. Please download this form, complete it,
and return it to us by fax or postal mail.

Mammography Scheduling
Screening mammograms can be scheduled either online or by phone at St. Joseph Mercy Health System locations. You will need a mammography requisition before your appointment, which our patients can obtain from our office.
Mammography appointments are available in Brighton, Canton, Howell, Plymouth, Saline, and Ypsilanti.

Low income, uninsured or underinsured?
BFFL may be able to help! A nurse arranges your mammogram and gives you information on taking good care of your breasts and it's FREE!
